INITIAL ENROLLMENT FEE             $100.00 (non-refundable)

ANNUAL RE-ENROLLMENT FEE   $50.00 (due on anniversary date)

PAST DUE CHARGES

Weekly payment (1 day late)   $2.50/day

Monthly payment (10 days late)   $10.00/month

Daily pickup (after 6:00 pm )   $2.00/minute

INSUFFICIENT CHECK CHARGE: $30.00 plus bank charges
