PRESCHOOL/CHILD CARE
ADMISSION PROCEDURES:

1. The parent/guardian has an initial interview with the Child Care Director or Director of Education.
   
   
2. The child may be requested to participate in the program for one-half day for evaluation and appropriate placement.
   
   
3. Completed documentation and non-refundable registration fee is presented by the parent/guardian:

• Application
• Medical Record (signed by a doctor)
• Immunization Records
• Vision and Hearing test results (when applicable)
• Background Information form
• Water Play Permission form
• Parent Acknowledgement of "Parent Guide to Child Care"
• Financial Contract
