ELEMENTARY SCHOOL ADMISSION PROCEDURES:

1. Parent/Guardian has initial interview and tour of campus with the Director of Education.
   
   
2. Appointment is made for student evaluation, and evaluation fee is paid.
   
   
3. Parent/Guardian is notified of acceptance by the Director of Education.
   
   
4. Completed documentation and non-refundable registration fee is presented by Parent/Guardian.

• Background Information
• Medical Evaluation with Doctor's signature
• Immunization Records
• Vision and Hearing Screening Results
• Photocopy of Birth Certificate
• Child's Social Security Number
• Financial Contract
• Request for Records from previous school (if applicable)
• Extended Care Agreement (if applicable)
• Statement from parent: "Why I want my child to attend CCL School"
